{"id":55948,"date":"2024-12-21T10:03:41","date_gmt":"2024-12-21T09:03:41","guid":{"rendered":"https:\/\/drgonzalezcantero.com\/psoriasis-control-comorbidities\/"},"modified":"2026-09-03T13:48:32","modified_gmt":"2026-09-03T11:48:32","slug":"psoriasis-control-comorbidities","status":"publish","type":"post","link":"https:\/\/drgonzalezcantero.com\/en\/psoriasis-control-comorbidities\/","title":{"rendered":"Does controlling psoriasis reduce the risk of developing comorbidities?"},"content":{"rendered":"\n<p class=\"wp-block-paragraph\"><em>In this article, I explain to what extent good psoriasis control may be relevant to the risk of associated comorbidities and why comprehensive care is important for maintaining overall health.<\/em><\/p>\n\n<p class=\"wp-block-paragraph\">Psoriasis is a chronic <a href=\"https:\/\/medlineplus.gov\/spanish\/autoimmunediseases.html\" target=\"_blank\" rel=\"noreferrer noopener\">immune-mediated<\/a> inflammatory disease. Although it is most visible through skin manifestations such as red, scaly plaques, its clinical relevance can extend beyond the skin. People with psoriasis have a higher prevalence of a number of associated conditions, or comorbidities, including psoriatic arthritis, cardiovascular disease, obesity, metabolic syndrome, type 2 diabetes, fatty liver disease and certain psychological disorders. <\/p>\n\n<p class=\"wp-block-paragraph\">But can controlling psoriasis itself help reduce these risks? Current evidence suggests that achieving good control of inflammatory disease is important, but the relationship between psoriasis treatment and the prevention of comorbidities is complex. Good psoriasis management should therefore form part of a broader strategy that also includes the identification and direct management of cardiovascular, metabolic, musculoskeletal and psychological risk factors. In this article, I explain why a comprehensive approach to psoriasis is important and how appropriate treatment, monitoring and prevention can contribute to better long-term health.  <\/p>\n\n<h2 class=\"wp-block-heading\">Psoriasis as a disease with systemic implications<\/h2>\n\n<p class=\"wp-block-paragraph\">To understand the relationship between psoriasis and comorbidities, it is useful to consider the inflammatory mechanisms involved in the disease. Psoriasis results from dysregulation of the immune system, with inflammatory pathways involving cytokines such as tumour necrosis factor alpha (TNF-\u03b1) and the <a href=\"https:\/\/es.wikipedia.org\/wiki\/Interleucina\">interleukins<\/a> IL-17 and IL-23 playing an important role.  <\/p>\n\n<p class=\"wp-block-paragraph\">The impact of psoriasis control on systemic inflammation  <\/p>\n\n<h2 class=\"wp-block-heading\">The impact of psoriasis control on systemic inflammation<\/h2>\n\n<p class=\"wp-block-paragraph\">One of the principal objectives of psoriasis treatment is to reduce disease activity and inflammation. Achieving good control can substantially improve skin symptoms, physical discomfort and quality of life. Systemic and biological therapies can also reduce inflammatory activity beyond visible skin lesions.  <\/p>\n\n<p class=\"wp-block-paragraph\">An important area of research is whether reducing psoriasis-related inflammation can also translate into a lower long-term risk of some associated diseases, particularly cardiovascular disease. Observational studies and mechanistic research have produced encouraging findings, including reductions in inflammatory biomarkers and changes in vascular inflammation in some patients receiving effective systemic treatment. However, this does not yet establish that treating psoriasis alone prevents cardiovascular events or other comorbidities. Further prospective studies and clinical trials are needed. For this reason, controlling psoriasis should complement \u2014 rather than replace \u2014 direct assessment and management of established cardiovascular and metabolic risk factors.  <\/p>\n\n<figure class=\"wp-block-image size-full\"><img decoding=\"async\" width=\"914\" height=\"571\" src=\"https:\/\/drgonzalezcantero.com\/wp-content\/uploads\/2024\/12\/comorbilidas-relacionadas-psoriasis-control-01.jpg\" alt=\"Psoriasis-associated comorbidities and the importance of comprehensive disease management.\" class=\"wp-image-54774\" title=\"Psoriasis-associated comorbidities and the importance of comprehensive disease management.\" srcset=\"https:\/\/drgonzalezcantero.com\/wp-content\/uploads\/2024\/12\/comorbilidas-relacionadas-psoriasis-control-01.jpg 914w, https:\/\/drgonzalezcantero.com\/wp-content\/uploads\/2024\/12\/comorbilidas-relacionadas-psoriasis-control-01-480x300.jpg 480w\" sizes=\"(min-width: 0px) and (max-width: 480px) 480px, (min-width: 481px) 914px, 100vw\" \/><\/figure>\n\n<h2 class=\"wp-block-heading\">Psoriasis-associated comorbidities and the importance of comprehensive management<\/h2>\n\n<p class=\"wp-block-paragraph\"><strong>1. Cardiovascular disease<\/strong><\/p>\n\n<p class=\"wp-block-paragraph\">Cardiovascular disease is one of the most extensively studied areas in relation to psoriasis. People with psoriasis, particularly those with more severe disease, may have a higher cardiovascular risk than the general population. Shared factors include systemic inflammation as well as conventional cardiovascular risk factors such as obesity, hypertension, smoking, diabetes and abnormal lipid levels. Atherosclerosis, which involves changes in the arterial wall and the development of atherosclerotic plaques, is a major underlying process in cardiovascular disease. <\/p>\n\n<p class=\"wp-block-paragraph\">Effective psoriasis treatment can reduce systemic inflammatory markers, including C-reactive protein in some patients. Research is also investigating whether certain systemic and biological treatments may influence vascular inflammation and long-term cardiovascular outcomes. Nevertheless, psoriasis control should not be considered a substitute for cardiovascular prevention. Blood pressure, cholesterol, glucose metabolism, smoking, weight, physical activity and other risk factors should be assessed and managed directly when appropriate. <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>2. Type 2 diabetes and metabolic syndrome<\/strong><\/p>\n\n<p class=\"wp-block-paragraph\">Psoriasis is also associated with an increased prevalence of obesity, insulin resistance, type 2 diabetes and metabolic syndrome. The relationship is complex and may involve shared inflammatory pathways, genetic factors and lifestyle-related risks. Some studies have examined whether effective psoriasis treatment is associated with changes in metabolic parameters. However, evidence is not sufficient to conclude that controlling psoriasis itself prevents diabetes or metabolic syndrome. For this reason, metabolic risk should be assessed independently. Maintaining a healthy weight, following an appropriate dietary pattern, exercising regularly and identifying abnormalities in blood glucose, blood pressure and lipid levels remain important parts of comprehensive psoriasis care.  <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>3. Psoriatic arthritis<\/strong><\/p>\n\n<p class=\"wp-block-paragraph\">Psoriatic arthritis is one of the most important comorbidities associated with psoriasis. It is a chronic inflammatory musculoskeletal disease that can affect joints, tendons and other structures. If active inflammation is not adequately treated, some patients may develop irreversible structural joint damage and impaired function. Early identification is therefore particularly important. Symptoms such as persistent joint pain or swelling, morning stiffness, swollen fingers or toes, or pain at tendon insertions should prompt further assessment. <\/p>\n\n<p class=\"wp-block-paragraph\">Several systemic and biological treatments can be effective for both psoriasis and psoriatic arthritis because they act on inflammatory pathways involved in both diseases. However, good control of skin psoriasis cannot currently be assumed to prevent psoriatic arthritis from developing. Whether particular psoriasis treatments can modify this risk remains an important area of research. Once psoriatic arthritis is present, appropriate rheumatological and dermatological management is essential to control inflammation and reduce the risk of joint damage. <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>4. Fatty liver disease and psoriasis: an important association<\/strong><\/p>\n\n<p class=\"wp-block-paragraph\">Fatty liver disease involves the accumulation of fat within liver cells and can, in some patients, progress to more advanced forms of liver disease. It is more common in people with psoriasis, particularly when metabolic risk factors such as obesity, insulin resistance, type 2 diabetes or metabolic syndrome are also present. <\/p>\n\n<p class=\"wp-block-paragraph\">The relationship between psoriasis and fatty liver disease is probably multifactorial. Systemic inflammation may play a role, but metabolic factors are also particularly important. Obesity, insulin resistance and metabolic syndrome \u2014 which are more prevalent in some populations with psoriasis \u2014 substantially influence the risk of fatty liver disease.  <\/p>\n\n<p class=\"wp-block-paragraph\">Research has investigated whether systemic psoriasis therapies may improve certain inflammatory or metabolic markers associated with liver disease. These findings remain under study and should not be interpreted as evidence that psoriasis treatment alone prevents or treats fatty liver disease. Lifestyle measures such as maintaining a healthy weight, following a balanced dietary pattern and exercising regularly are important for metabolic and liver health.  <\/p>\n\n<p class=\"wp-block-paragraph\">Patients with psoriasis who have relevant risk factors may benefit from assessment for fatty liver disease according to their individual clinical situation. Comprehensive management aims not only to control the skin disease but also to identify and address conditions that may affect long-term health. <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>5. Depression and anxiety<\/strong><\/p>\n\n<p class=\"wp-block-paragraph\">Psoriasis can have a significant psychological impact. Visible lesions, symptoms such as itching or pain, unpredictability of flare-ups and the effect of the disease on work, relationships and everyday activities can all affect quality of life and emotional wellbeing. Research has also investigated possible biological links between inflammation and depression, although the relationship is complex and cannot be explained by inflammation alone. <\/p>\n\n<p class=\"wp-block-paragraph\">Achieving better psoriasis control may improve psychological wellbeing by reducing symptoms and the burden of the disease. However, depression and anxiety should also be recognised and treated directly when present. Psychological support and appropriate mental healthcare may form an important part of comprehensive psoriasis management. <\/p>\n\n<figure class=\"wp-block-image size-full\"><img decoding=\"async\" width=\"914\" height=\"571\" src=\"https:\/\/drgonzalezcantero.com\/wp-content\/uploads\/2024\/12\/importancia-enfoque-integral-01.jpg\" alt=\"The importance of a comprehensive approach to psoriasis management.\" class=\"wp-image-54773\" title=\"The importance of a comprehensive approach to psoriasis management.\" srcset=\"https:\/\/drgonzalezcantero.com\/wp-content\/uploads\/2024\/12\/importancia-enfoque-integral-01.jpg 914w, https:\/\/drgonzalezcantero.com\/wp-content\/uploads\/2024\/12\/importancia-enfoque-integral-01-480x300.jpg 480w\" sizes=\"(min-width: 0px) and (max-width: 480px) 480px, (min-width: 481px) 914px, 100vw\" \/><\/figure>\n\n<h2 class=\"wp-block-heading\">The importance of a comprehensive approach to treatment<\/h2>\n\n<p class=\"wp-block-paragraph\">Psoriasis treatment should be adapted to the severity, location and impact of the disease, as well as to previous treatment response and the individual patient\u2019s clinical circumstances. Topical treatments and phototherapy may be appropriate for selected patients, while systemic or biological therapies may be considered when clinically indicated. The choice of treatment is not based exclusively on the percentage of skin affected. High-impact locations, poor control with topical treatment, quality of life and other clinical factors can also influence treatment decisions. Systemic and biological therapies can provide effective control of psoriasis and, depending on the treatment selected, may also be appropriate when certain associated inflammatory diseases are present. However, they should not be prescribed solely on the assumption that they will prevent every psoriasis-associated comorbidity. <\/p>\n\n<p class=\"wp-block-paragraph\">A comprehensive approach also includes addressing modifiable health risks. The Mediterranean diet is a balanced dietary pattern with recognised cardiovascular and metabolic benefits and is one of the dietary approaches most extensively studied in people with psoriasis. Regular physical activity, healthy weight management, good sleep and appropriate attention to psychological wellbeing are also relevant.  <\/p>\n\n<p class=\"wp-block-paragraph\">Avoiding smoking and limiting excessive alcohol consumption are particularly important for overall cardiovascular, metabolic and liver health. <\/p>\n\n<p class=\"wp-block-paragraph\">A <a href=\"https:\/\/drgonzalezcantero.com\/en\/comprehensive-psoriasis-treatment\/\">comprehensive approach to psoriasis<\/a> takes these different dimensions into account, with the aim of achieving good disease control while also identifying and managing factors that may affect the patient\u2019s overall health.<\/p>\n\n<h2 class=\"wp-block-heading\">Prevention as a key part of managing psoriasis-associated comorbidities<\/h2>\n\n<p class=\"wp-block-paragraph\">Not all comorbidities associated with psoriasis can be prevented. For this reason, prevention should not rely exclusively on achieving clear skin or reducing psoriasis activity. A more appropriate strategy combines: effective treatment of psoriasis; early identification of associated conditions; assessment of cardiovascular and metabolic risk factors; appropriate lifestyle measures; regular clinical follow-up; and referral to other specialists when necessary. Collaboration between dermatologists, rheumatologists, cardiologists, primary care physicians and other specialists can be particularly valuable when a patient has specific comorbidities or risk factors.  <\/p>\n\n<h2 class=\"wp-block-heading\">Conclusion<\/h2>\n\n<p class=\"wp-block-paragraph\">Good psoriasis control is important for reducing the burden of the disease, improving symptoms and supporting quality of life. Because psoriasis is associated with a number of inflammatory, cardiovascular, metabolic, musculoskeletal and psychological conditions, its management should also include appropriate assessment of these broader health risks. Reducing psoriasis-related inflammation may have beneficial effects beyond the skin, and this remains an important area of research. However, current evidence does not allow us to conclude that controlling psoriasis alone will prevent all associated comorbidities. <\/p>\n\n<p class=\"wp-block-paragraph\">The most appropriate approach is therefore a comprehensive one: treat the psoriasis effectively, identify associated conditions early and manage each patient\u2019s cardiovascular, metabolic and other risk factors directly. Treating psoriasis is not simply about improving its visible manifestations. It is part of a wider approach to protecting health and reducing the overall impact of the disease over time. <\/p>\n\n<p class=\"wp-block-paragraph\"><em>I invite you to share this article about
