As a dermatologist specialising in psoriasis, I regularly see the impact that a persistent and sometimes unpredictable disease can have on patients, particularly when symptoms recur or remain inadequately controlled. <\/p>\n\n<p class=\"wp-block-paragraph\">In this article, I explain some of the factors that contribute to the chronic nature of psoriasis. Understanding how the disease behaves and how it is treated can help patients take a more informed role in its long-term management. Although psoriasis is a <a href=\"https:\/\/es.wikipedia.org\/wiki\/Enfermedad_cr&#xF3;nica\" target=\"_blank\" rel=\"noreferrer noopener\">chronic disease<\/a>, effective treatment can achieve a high level of disease control in many patients.<\/p>\n\n<h2 class=\"wp-block-heading\">Genetic factors<\/h2>\n\n<p class=\"wp-block-paragraph\">Genetic predisposition is one of the most important factors involved in psoriasis. Studies have shown that psoriasis is more common among people who have affected family members, indicating an important hereditary component. Researchers have also identified a number of genetic variants associated with the disease, including variants within the <a href=\"https:\/\/es.wikipedia.org\/wiki\/Complejo_mayor_de_histocompatibilidad\" target=\"_blank\" rel=\"noreferrer noopener\">major histocompatibility complex (MHC)<\/a>, which plays an important role in immune regulation.  <\/p>\n\n<p class=\"wp-block-paragraph\">These genetic factors influence susceptibility to psoriasis and may also contribute to differences in how the disease presents. However, having genetic susceptibility does not mean that a person will necessarily develop psoriasis. Genetics is therefore one important part of understanding why psoriasis can persist throughout life, but it acts alongside immune and environmental factors. <\/p>\n\n<h2 class=\"wp-block-heading\">Immune-mediated nature of psoriasis<\/h2>\n\n<p class=\"wp-block-paragraph\">Psoriasis is an <strong>immune-mediated inflammatory disease<\/strong>. In psoriasis, dysregulation of the immune system activates inflammatory pathways that cause skin cells to proliferate more rapidly than usual. Immune cells, including T cells, and inflammatory mediators such as cytokines play an important role in this process. <\/p>\n\n<p class=\"wp-block-paragraph\">This sustained <strong>immune activation<\/strong> contributes to the characteristic inflamed, red and scaly plaques associated with psoriasis. The immune-mediated nature of the disease is also one of the reasons why many psoriasis treatments target specific components of the immune response. The aim is to reduce inappropriate inflammatory activity while maintaining an appropriate balance between efficacy and safety. <\/p>\n\n<h2 class=\"wp-block-heading\">A disease with a fluctuating course<\/h2>\n\n<p class=\"wp-block-paragraph\">Psoriasis often has a <strong>fluctuating course<\/strong>. Disease activity may increase and decrease over time. Some people experience periods in which symptoms become considerably less noticeable or even disappear, while at other times psoriasis may become more active again.  <\/p>\n\n<p class=\"wp-block-paragraph\">This pattern of <strong>periods of activity and relative remission<\/strong> is characteristic of many people with psoriasis. The course of the disease varies greatly between individuals, and it is not always possible to predict when a flare-up will occur. Understanding this variability can help patients and healthcare professionals develop more appropriate long-term treatment and follow-up strategies.<\/p>\n\n<h2 class=\"wp-block-heading\">Psoriasis is a chronic disease: what factors can influence flare-ups?<\/h2>\n\n<p class=\"wp-block-paragraph\">Several factors may influence psoriasis activity or be associated with flare-ups in susceptible individuals. <strong>Environmental factors<\/strong>: Cold and dry weather may worsen dryness and discomfort in some people with psoriasis, particularly when the skin barrier is not adequately supported. Natural ultraviolet exposure may improve psoriasis in some patients, but this does not mean that uncontrolled sun exposure should be used as a treatment. Sunburn should always be avoided, appropriate photoprotection remains important, and medically supervised phototherapy is different from deliberate exposure to natural sunlight.  <\/p>\n\n<figure class=\"wp-block-image aligncenter size-full\"><img decoding=\"async\" width=\"960\" height=\"540\" src=\"https:\/\/drgonzalezcantero.com\/wp-content\/uploads\/2024\/07\/clima-frio-seco-empeora-psoriasis-01.jpg\" alt=\"Cold and dry weather may worsen psoriasis symptoms in some patients.\" class=\"wp-image-54648\" srcset=\"https:\/\/drgonzalezcantero.com\/wp-content\/uploads\/2024\/07\/clima-frio-seco-empeora-psoriasis-01.jpg 960w, https:\/\/drgonzalezcantero.com\/wp-content\/uploads\/2024\/07\/clima-frio-seco-empeora-psoriasis-01-480x270.jpg 480w\" sizes=\"(min-width: 0px) and (max-width: 480px) 480px, (min-width: 481px) 960px, 100vw\" \/><\/figure>\n\n<p class=\"wp-block-paragraph\"><strong>Stress<\/strong>: Many patients report an association between periods of significant stress and worsening psoriasis. Scientific studies on stress as a direct trigger have produced mixed results, and its effect varies considerably between individuals. Nevertheless, psychological wellbeing is relevant to quality of life and may be an important part of comprehensive psoriasis care. <strong>Infections<\/strong>: Certain infections are well-recognised triggers for particular forms of psoriasis. For example, streptococcal throat infection is associated with the onset of guttate psoriasis, particularly in <a href=\"https:\/\/drgonzalezcantero.com\/en\/childhood-psoriasis\/\">children<\/a>. <strong>Medicines<\/strong>: Some medicines have been associated with the onset or worsening of psoriasis in susceptible individuals. However, prescribed medication should never be stopped or changed without medical advice. If a medicine is suspected of affecting psoriasis, this should be reviewed with the prescribing doctor and dermatologist.  <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Hormonal changes<\/strong>: Hormonal changes, including those occurring during puberty, pregnancy or <a href=\"https:\/\/drgonzalezcantero.com\/psoriasis-mujeres\/\">menopause<\/a>, may coincide with changes in psoriasis activity in some patients. The effect is highly individual and is not the same in every person or at every stage of life. <strong>Koebner phenomenon<\/strong>: The Koebner phenomenon refers to the appearance of psoriasis lesions in previously unaffected skin after trauma. Cuts, scratches, friction or burns can sometimes trigger lesions in susceptible individuals. <\/p>\n\n<p class=\"wp-block-paragraph\"><strong>Lifestyle factors<\/strong>: Certain lifestyle factors, particularly smoking and excessive alcohol consumption, are associated with poorer health outcomes and may also be relevant to psoriasis severity or disease control.<\/p>\n\n<p class=\"wp-block-paragraph\">These factors do not explain the <strong>chronic nature of psoriasis<\/strong> on their own, but they may influence how the disease behaves in individual patients. Understanding them can help form part of a broader strategy for long-term management.<\/p>\n\n<h2 class=\"wp-block-heading\">Although there is no definitive cure, psoriasis can be controlled<\/h2>\n\n<p class=\"wp-block-paragraph\">Despite major advances in psoriasis research, there is currently <strong>no definitive cure<\/strong> for the disease. Treatment therefore focuses on achieving and maintaining <strong>disease control<\/strong>, reducing inflammation, relieving symptoms and limiting the impact of psoriasis on everyday life. Available options include topical treatments, phototherapy, systemic medicines and advanced targeted therapies, including biological treatments. <\/p>\n\n<p class=\"wp-block-paragraph\">The appropriate treatment depends on factors such as disease severity and location, its impact on quality of life, previous treatment response, comorbidities and the individual patient\u2019s circumstances. The absence of a definitive cure means that psoriasis may require long-term follow-up and, in many cases, ongoing or intermittent treatment. However, current treatments can achieve <strong>substantial and sustained disease control in many patients<\/strong>, including prolonged periods with very limited or no visible skin involvement.<\/p>\n\n<p class=\"wp-block-paragraph\">Research continues to investigate the biological mechanisms of psoriasis and new approaches to treatment, with the aim of <strong>further improving<\/strong> long-term disease control, safety and quality of life.<\/p>\n\n<h2 class=\"wp-block-heading\">Why is it important to understand psoriasis as a chronic disease?<\/h2>\n\n<p class=\"wp-block-paragraph\">Psoriasis is a chronic disease influenced by genetic, immune, environmental and individual factors. Understanding this helps both patients and dermatologists approach management with <strong>realistic long-term objectives<\/strong>. It also helps explain why treatment may need to be adjusted over time and why <strong>disease control<\/strong> should not be assessed only at a single point. <\/p>\n\n<p class=\"wp-block-paragraph\">As a dermatologist and researcher specialising in psoriasis, I consider <strong>education and a clear understanding<\/strong> of the disease essential. Knowing why psoriasis behaves as it does, what may influence it and which treatment options are available can help patients make informed decisions and work more effectively with their healthcare team. The objective is not simply to treat individual flare-ups, but to achieve sustained control and reduce the impact of psoriasis on quality of life over the long term.<\/p>\n\n<p
